Health Survey Questionnaire

Eating disorders include a range of conditions in which food, weight, and appearance dominate one’s life. This preoccupation interferes with everyday living as well as physical and mental health.

Eating disorders are not a choice. They evolve due to a combination of biological, psychological, and sociocultural factors and often coexist with other issues such as depression, anxiety, and PTSD.

Eating disorders have the second highest mortality rate of all mental illnesses. This questionnaire can help you determine if you should seek professional help. It does not take the place of a clinical assessment. If you answer yes to any of the following questions, please consider talking with a trained professional.
